How to make a referral
You must meet the following criteria:
- history of non-engagement.
- homeless or at risk of becoming homeless
- alcohol dependent.
All referrals should include the named Care Manager and should be sent to the Service Commissioning Officer, Kim McDonald. The referral will be considered and passed on to our Health and Wellbeing team.
If a referral is suitable, a Health and Wellbeing Team Leader will contact the relevant Care Manager and arrange an initial contact visit.
The service provides 12-hour support time per week to each individual and includes travel and administration. We work with a waiting list.
